Please report your student's absence within 3 days of your student returning to school. Please provide documentation from your physician when possible, especially for extended illness. Use this form to report student vacations, trips, or emergency situations that may lead to an absence.
While you can inform your teacher via email/Class Dojo when your student is out, completing this form is necessary for the office staff to change absences to excused. Please note absences cannot be called into the office by phone. All absences must be reported using this form.
